CORONAVIRUS  (COVID-19) UPDATE: 12/03/20

If you have a pre-booked appointment at the surgery over the next few days, please contact us BY TELEPHONE in advance of this for advice if you develop any of the following symptoms - cough, cold, fever, shortness of breath - OR if you have recently returned from an area affected by coronavirus OR if you have been in contact with someone who has tested positive for Coronavirus.

 

If you may have been exposed to coronavirus DO NOT ATTEND your GP or hospital emergency department, please visit https://111.nhs.uk/covid-19 for further advice, or call NHS 111.

Online booking of fact to face appointments has been suspended for the foreseeable future to ensure that patients are triaged to the right service and no-one is put at risk of Coronavirus.

 

New Patient Registration

New patient registrations during the Coronavirus period:

We are not accepting new patient registrations in person at the present time.  You can download the registration form and medical questionnaire, plus all other relevant information here on the website by clicking the New Patient Registrations tab above.  Please complete and return by post or online and if you are able please also scan/upload your ID.  If you have any queries or problems registering online, then please contact the surgery on 0161 773 0494 

Please apply in person at reception during opening hours. Please bring in the completed and signed medical cards for each person requesting to join the practice if available. If you have previously registered with a GP in the United Kingdom you must bring in your National Health Service Number. Your records will be applied for through the Health Authority. If you want to use our online services, proof of identification must be shown.

You will be invited to attend for a new patient medical examination and will not be fully registered with the practice until you have had this medical.

The practice may decline an application if you reside outside the practice area or if the list is closed. You will not be refused for any other reason. You will be asked for proof of identity and residence.
