Practice Information Longfield Medical Practice Telephone: 0161 773 0494 Prescription Telephone Line: There is a special telephone line for repeat prescriptions, open from 9.00am to 12 noon, Monday - Friday Surgery Opening Times Monday to Friday, 8.30am-6.30pm Home Visits Home visits are made to patients who cannot attend the surgery for a medical reason. The doctor on duty will ring you to discuss if a home visit is the most appropriate management for you. The receptionist will need to record a brief reason for the visit to enable the doctor to prioritise the call. If possible, please telephone before 10.30 am if you think you may need a home visit. OUT-OF-HOURS EMERGENCY GP SERVICE When the Medical Practice is closed there is an emergency doctor service available. Bury Primary Care Trust is responsible for commissioning this service.
How to register with this practice Please apply in person at reception during opening hours. Please bring in the completed and signed medical cards for each person requesting to join the practice if available. If you have previously registered with a GP in the United Kingdom you must bring in your National Health Service Number. Your records will be applied for through the Health Authority. You will be invited to attend for a new patient medical examination and will not be fully registered with the practice until you have had this medical. The practice may decline an application if you reside outside the practice area or if the list is closed. You will not be refused for any other reason. You will be asked for proof of identity and residence. How you see one of the Doctors The practice runs clinics by appointment. Each appointment is for 10 minutes. You can book an appointment by phone (0161 773 0494) or by calling in to the surgery. In the event of an emergency every effort will be made to enable you to see a doctor on the same day.
Laboratory tests and X-rays Please contact the surgery for your test results only between 2pm and 5.30pm. Results cannot be given at any other time. Practice Complaints Procedure If you have any complaints or concerns about the service you have received from this practice our practice manager will be pleased to help you. We hope that most problems can be sorted out easily and quickly, often at the time they arise, but if you wish to make a complaint please contact our practice manager who will deal with your complaint and explain the procedure.
